Health Care Operations
To support administrative, quality improvement, training, and auditing activities necessary to maintain compliance and service effectiveness.

Required by Law
We may disclose PHI when required by law, such as reporting abuse or neglect, responding to court orders, or meeting public health requirements.

Other Permitted Uses
We may disclose limited PHI for appointment reminders, service coordination, or to prevent a serious threat to health or safety.

3. Your Rights Regarding Your Health Information

You have the right to:

  • Inspect and request a copy of your health information.

  • Request a correction if information is inaccurate or incomplete.

  • Request restrictions on how your information is used or shared (we may not always be able to agree).

  • Request confidential communication (e.g., alternate phone or email).

  • Receive an accounting of disclosures (list of when we shared your PHI).

  • Receive a paper or electronic copy of this notice upon request.
